| Dohle's Kennel 816-628-6980 PUPPY PURCHASE AGREEMENT I (buyer's name)_____________________________________________agree to purchase one (1) AKC _____________________________________(male) (female) (Cocker Spaniel ) ( Shih Tzu ) puppy with full AKC registration from Dohle’s Kennel. If shipped, puppy will be shipped on Northwest or another available airline for additional $275.00 . This document also acts as a bill of sale and /or transfer of ownership from Dohle 's Kennel to the name above when all transfers and transactions are made. PURCHASER MUST PAY IN FULL BEFORE RECEIVING PUPPY. Health Guarantee The puppy is received in good health condition and comes with a 72 hour health guarantee that goes into effect at the time the new owner’s take possession of the puppy. All puppies have received a ” well inspection” by a licensed veterinarian. They have received their first shots and worming before going to their new home. This puppy is not warranted against cherry eye or allergies. Absolutely no refunds will be given. If veterinarian documents that puppy suffers from a life threatening condition, a replacement puppy will be given (Of same or similar color, sex and value as soon as is possible) Provided proper medical certification is provided. Purchaser must submit two legal and authenticated diagnosis conclusions: One Diagnosis each from two non-associated licensed veterinarians located in the buyers claimed state of occupancy. My Vet must verify condition. Purchaser must return puppy to Dohle's Kennel for replacement , Dohle's Kennel will not be responsible for any incurred Vet fees or shipping cost during your ownership of your Shih Tzu/Cocker Spaniel. Purchaser must submit records of verification of vet exams, and puppy’s vet checks, and continued shot and de-worming records in an appropriate time frame. Veterinarian and other costs are absolutely Non refundable we are not responsible for any incurred bills related to any congenital defects or other health or maintenance issues after purchaser takes possession of puppy. This guarantee does not cover problems related to non con congenital diseases (non genetic, or non life threatening). Learn all you can about your breed of puppy before making a purchase. Their environment in their new home is important to good health . Puppy proof your house, do not take them into public places, do not take them around other animals for the first couple of weeks. Make sure they have a clean environment free of problem causing bacteria. Take your puppy to a veterinarian for an exam. This agreement is void if not done within 72 hours of purchase. You will need to continue with a worming program and shots. It is extremely important to complete the puppy’s vaccinations and socialization. I understand that all sales are final and that no refunds will be given after the expiration of the health guarantee. Don’t change your puppy’s food drastically as this can cause diarrhea & sickness, claimed sickness due to foods or feedings are also not covered in the guarantee. All breeds are predisposed to a variety of health issues that are susceptible as puppies, buyer is responsible for any treatment of these conditions. Dohle’s Kennels is not responsible for any circumstance that may arise from the shipping of this puppy. I have fully read and understood the above agreement. I accept the above agreement.
